Plan of Correction Review and Consultation
When we are assigned to draft and present a corrective plan for assisted living and memory care facilities, we adhere to an organized process. This process guarantees that the plan tackles any detected deficiencies, offers clear solutions, and shows the facility's dedication to improvement.
Our Comprehensive Process
1Initial Evaluation and Examination of Deficiencies
Prior to drafting the plan of correction (PoC), we perform a thorough examination of the identified deficiencies. This includes meticulous inspection report analysis, determining root causes through staff and management interviews, policy reviews, and facility walk-throughs, and engaging with facility leadership to ensure all stakeholders understand the issues and the facility's dedication to resolving them.
2Drafting the Plan of Correction
After completing the assessment, we move on to drafting the PoC. Each section includes:
- Cover Letter and Introduction: Deficiency summary, facility details, and commitment statement
- Detailed Action Plan: Problem statements, corrective actions, timelines, and responsible parties
- Evaluation and Monitoring: Continuous improvement plans, regular audits, and feedback mechanisms
- Staff Accountability: Clear accountability structures and reporting procedures
- Training and Development: Ongoing education plans and effectiveness evaluation
3Finalizing and Submitting the Plan
Once the PoC has been drafted, we finalize the document by reviewing it for clarity, accuracy, and thoroughness. We seek additional input from facility management prior to submission to the facility Administrator. The plan is then submitted to the relevant regulatory body within the required timeframe, typically within 10 to 30 days after receiving a citation or deficiency report.
4Presentation of the Plan
After submitting the PoC, a presentation may be required for internal staff, the governing body, or regulatory officials. The presentation aims to ensure everyone understands the actions being taken and supports the plan.
Internal Presentation: Review of deficiencies, walk-through of corrective actions, explanation of monitoring procedures
External Presentation: Demonstration of commitment to correction, detailed corrective actions, timelines and accountability
5Follow-up and Continuous Monitoring
Once the plan is implemented, facility management and ourselves will closely monitor progress through scheduled follow-up visits, audits to verify proper implementation, necessary adjustments based on findings, and ongoing reporting to regulatory agencies and internal stakeholders.
