Position Overview
Benevolent Healthcare Solutions is seeking an enthusiastic and compassionate Community Engagement Case Manager to help clients build meaningful connections within their communities. In this rewarding role, you'll support clients in discovering their interests, participating in activities, and developing the social skills and confidence needed for independent community engagement.

Key Responsibilities
- Assess client interests, abilities, and community integration goals
- Facilitate participation in community activities, events, and programs
- Support clients in developing social skills and building relationships
- Coordinate transportation and provide accompaniment to activities
- Help clients explore volunteer and employment opportunities
- Document client progress and maintain accurate records
- Collaborate with care teams and families to support client goals
- Build partnerships with community organizations and resources
Qualifications
- Bachelor's degree in Social Work, Human Services, or related field preferred
- Experience working with individuals with disabilities or special needs
- Strong interpersonal and communication skills
- Creative problem-solving and planning abilities
- Knowledge of community resources and programs
- Valid driver's license and reliable transportation
- Ability to work flexible hours including some evenings/weekends
- Patience, empathy, and genuine passion for helping others
